Fill-in general information, making sure to include date, your name and contact information, the type of information you are requesting and the incident information to the best of your knowledge.
Fax your completed form to the fax number on the top of the form.
The standard processing time is usually 3 days to 3 weeks, depending on the details of the information requested, its accessibility and whether or not the incident is a criminal case as well and your request requires authorization from the Marion County DA Office. There are no guarantees for processing time so please plan accordingly. The most effective way to submit or cancel a request is via fax to the number located on the top of the Request form. For questions or concerns call 503-982-2340 Ext. 480 messages are checked periodically. Cancellations made by phone may be subject to request processing fees.
Once the Information Request is complete you will be notified, via the telephone number on the request form, of its completion and the associated cost. You will also be provided with instructions for pick-up or you may send a check for the total cost to: NORCOM 911 1060 Mt. Hood Ave., Woodburn, OR 97071. We would then mail your information to you. All completed requests for non-User Agency personnel are subject to processing fees. Payment is required whether the information requested is collected or not.
The NORCOM Information Request Form is for information related to a North Marion County 9-1-1 Emergency or Non-Emergency phone call. Other information related to an incident may be solicited from the responding police, fire or medical agencies.
